Agency Reporting

All agencies are required to report the number of individuals they serve. This data helps us in a variety of ways. The reports can now be filled out monthly and are due at least every quarter (January 15, April 15, July 15 and October 15). Please complete the report using the form below or through Online Ordering. The Agency Reporting Instructions explain how to complete the report in Online Ordering.
